Admission Pack

Your doctor will have provided you with an admission pack that needs to be completed and returned to the hospital to confirm your booking.

The admission pack provides us with the information to finalise your booking and to let us know about any special care you may require. It provides you with information about your hospital bill and how you can pay your account and general information about the hospital to assist you whilst you are with us. It also provides you with information on how we protect your privacy and information on your rights and responsibilities whilst in hospital.

Tear off the forms along the perforated lines and return them to the hospital at least ten (10) working days before your admission unless coming to the pre admission clinic in which case please bring all forms to your pre admission appointment. Retain the rest of the package.
